Polymyalgia Rheumatica is an inflammatory disorder that causes muscle pain and stiffness, primarily in the shoulders and hips. It typically affects adults over age 50.

Symptoms

  • Aching and stiffness in shoulders, neck, and hips
  • Morning stiffness lasting an hour or more
  • Fatigue and low-grade fever
  • Difficulty with daily activities (dressing, rising from a chair)

Causes & Risk Factors

  • Aging immune system changes
  • Genetic predisposition
  • Association with Giant Cell Arteritis (GCA)

Diagnosis

  • Elevated ESR and CRP
  • Clinical history and physical exam
  • Rapid response to corticosteroids is a key diagnostic clue

Treatment Options

  • Joint pain and swelling (often in fingers and toes)
  • Morning stiffness
  • Nail pitting or separation
  • Lower back or heel pain
  • Red, scaly skin patches (psoriasis)

When to See a Rheumatologist

Sudden onset of shoulder and hip stiffness in older adults should be evaluated promptly—PMR responds best when treated early.

Most patients respond well to treatment and can eventually discontinue medication.”
